We are pleased that you and your employer have chosen City
County Insurance Services (CIS) as your employee benefits provider. This website
will provide you with the necessary information and forms to manage your health
care benefits.
If you are a new employee, you have the opportunity to complete the enrollment process
on-line. You will be directed to One Care Street to complete the Health Status Questionnaire
(HSQ). After completing the HSQ, the system will direct you to the benefits enrollment
screen. To begin the enrollment process, click on the “Your Benefits” icon above.
To assist you with the enrollment process, please print the
Instructions for New Employees.
If you are already enrolled in benefits, you can review your benefit selections
or notify your employer about “Life Events”, such as the birth/adoption of a child,
a marriage or divorce, a change in student status or a change of address, etc. To
manage your existing benefits, simply click on the “Your Benefits” icon above.
Why complete a Health Status Questionnaire (HSQ) and what is Healthy Benefits?
Where traditional insurance benefits protect employees when they are sick, Healthy
Benefits is focused on improving your health and keeping you well. Healthy Benefits
is a unique package of wellness services provided to you by your employer and CIS.
These services include, but not limited to, on-line medical resources, medical self-help
guides, wellness newsletters, “total-health” confidential coaching, and tobacco
cessation and weight management programs. In order to be eligible for the Healthy
Benefits programs and services, you must complete a Health Status Questionnaire
(HSQ). To learn more about staying healthy, or to complete a HSQ, click
on the "Healthy Benefits" icon above.
